Special Instructions:

Photo Requirements: Full face, both ears visible, Head occupies 70-80% of the photo, ,Eyes must be open, and a slight smile is allowed. Not allowed: wearing hats, sunglasses, masks, or any other items obstructing the face. Glasses are allowed, but lenses should not be reflective, and eyes must be clearly visible.

Agreement

By signing and submitting this form, you agreed and accepted the following:

  • Course Registration:

    • Participants must be at least 18 years of age or 12 years old with parental/guardian consent.

    • A completed and signed registration form is required for enrolment.

    • Full payment is due at the time of registration.

  • ​Health Requirements:

    • Participants must complete the "Dive Student Self Medical Assessment" and, if necessary, provide a medical clearance from a licensed physician.

    • Participants should be in good physical condition and able to swim.

  • ​Course Schedule:

    • The course schedule is subject to change based on weather conditions, instructor availability, and other unforeseen circumstances.

    • Participants must attend all scheduled classes, confined water sessions, and open water dives to successfully complete the course.

  • ​Refund and Cancellation Policy:

    • A full refund will be provided if the participant cancels at least 14 days before the course start date.

    • 50% refunds will be issued for cancellations made within 14 days of the course start date.

    • In the event of unforeseen circumstances, such as inclement weather or equipment issues, the course may be rescheduled, and refunds will be issued only if rescheduling is not possible.

  • ​Equipment:

    • ​Rental equipment for scuba diving must be returned in the same condition it was received. Any damage or loss will be the responsibility of the participant.

    • Maximum Loss / Damage Rate as follows:

  • ​Certification Requirements:

    • ​Participants must successfully complete all knowledge reviews, quizzes, pool sessions, and open water dives to receive certification.

    • Certification is contingent upon meeting all performance requirements and demonstrating safe diving practices.

  • ​Code of Conduct:

    • ​Participants are expected to follow all safety guidelines and instructions provided by the instructor.

    • Any participant found engaging in unsafe practices or behavior that poses a risk to themselves or others may be removed from the course without a refund.

  • ​Agreement and Release:

    • Participants must sign the "RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND EXPRESS ASSUMPTION OF RISK AGREEMENT" acknowledging the inherent risks of scuba diving and releasing the instructors and the dive centre from liability.

    • By enrolling in the scuba diving course, participants agree to abide by these terms and conditions. The dive centre reserves the right to modify or update these terms and conditions as needed. Participants will be notified of any changes in advance.

    • We reserved the right for final decision in event of any disputes.
